Instructions field: Retain and Reinforce for most issues with sewn structures, like loose hinges and damaged flapping spines.

If some pages are loose, R and R won't cut it.  Include "Careful loose pages" in instructions

Also include any other notes from Cataloging in instructions, such as "PocketOther notes from Cataloging

Bookplates: type these out on your typewriter, the preservation one has little ribbon left, and no correction tape.
